Illness and disease management week 4
There are a number of community and national resources available for people living with chronic illnesses.
One such resource is the Chronic Disease Coalition, which is a national nonprofit organization that provides support and advocacy for individuals living with chronic conditions. Through their website, they offer information on managing various diseases as well as connecting users to local resources in their respective communities.
The American Diabetes Association (ADA) is another helpful resource, providing education, support and advocacy to those affected by diabetes. The ADA offers various online programs that provide vital information on how to manage diabetes as well as support groups both online and in-person that can help connect individuals to other members of the same disease group.
Finally, many cities have local organizations specifically designed to meet the needs of those living with chronic illnesses such as Helping Others Manage Chronic Illness (HOMCI). HOMCI provides educational workshops on topics related to managing chronic illness in addition to individual counseling sessions for patients and family members seeking additional support or advice from medical professionals.
