Assignment: practicum – journal entry
During my experience in assessing and managing a patient and his family, the biggest “aha” moment I had was when I realized the patient’s disorder. The patient presented with numerous symptoms including fever, headache, nausea, fatigue, and confusion. After completing a thorough physical examination and ordering diagnostic testing, it became apparent that the underlying cause was meningitis. This diagnosis explained all of the symptoms he had been experiencing which allowed me to tailor an appropriate plan for treatment.
I also took into consideration the family dynamics during this process. It was important to ensure that they were kept informed about the situation as well as given instructions on how best to care for their loved one both during his hospital stay and post-discharge. Additionally I offered emotional support by offering reassurance addressing any questions or concerns they might have so everyone involved could feel more at ease here today .. After patient stabilized able eventually discharged home safe sound feeling more confident knowing had seen through entire process providing necessary resources order manage condition properly going forward no matter what particular challenges present themselves doing same .. All these things together helped further solidify my initial diagnosis leading greater level understanding overall here today benefit not only him but those who cared deeply about moving forward here today.
