In general, case management is defined as a service that assists individuals in navigating their physical, mental, and emotional healthcare needs. This includes providing information about treatment options, connecting patients to providers and insurance coverage, advocating for their rights within healthcare systems, and coordinating referrals between different medical specialists. Care management is similar but more comprehensive than case management; it takes a holistic approach to managing an individual’s health and well-being across all aspects of their life.
The theorist defining these concepts is Nelda Godfrey who defines case management as “the system of services that ensure the continuity of care among members of a patient’s health care team in order to provide coordinated resources for the patient” while care management is “the process of monitoring the effectiveness and efficiency of interventions used to manage the care plan established by the patient’s primary provider.”
