I have chosen a case study of an African American woman aged 25-45 with Type 2 Diabetes. She has been diagnosed with pre-diabetes (prediabetes) and is at risk for developing full-blown type 2 diabetes in the near future. The client’s current medications include Metformin and Januvia, which she takes on a daily basis. Her lab results indicate that she has slightly elevated blood sugar levels as well as high cholesterol, low HDL cholesterol, and mild increase in her triglycerides level. Additionally, the client is overweight (BMI of 30 kg/m2).
The goal of this nutritional care plan will be to assist the client in achieving better glycemic control by promoting healthy eating habits and lifestyle changes so that she can reduce her risk for developing type 2 diabetes or complications associated with it. Specifically, this plan will seek to provide strategies to improve dietary quality by decreasing processed foods, increasing nutrient dense foods such as vegetables, fruits, whole grains; lowering her cholesterol through diet modifications; and helping her reach a healthier weight through increased physical activity. The plan will also focus on providing adequate carbohydrate intake while monitoring glucose levels throughout the day in order to manage blood sugar fluctuations appropriately.
