Assessment:
The objective of this assessment is to develop and implement a change plan that will reduce the mortality and readmission rates for hospitalized patients at ABC Hospital over a specified period of time. In order to do this, it is necessary to first establish measurable goals for both outcomes as well as benchmark current performance levels so progress can be tracked over time and upcoming milestones identified prior to full implementation of the change plan.
For our initial goal, we will focus on reducing the mortality rate among ABC Hospital’s hospitalized patients by 10% within 6 months from its current level of 8.7%. To achieve this target, we must understand what factors may be contributing most significantly to patient deaths; this information can then be used to create targeted interventions that help address any identified issues or risks directly related to these factors.
The second goal set forth in this assessment is aimed at decreasing ABC Hospital’s readmission rates following hospital discharge by 15%, from its current level of 17.5%. Due to the complexities associated with post-discharge care management, it is imperative that effective communication between all involved parties – including healthcare professionals, family members/caregivers and patients themselves – occurs throughout all stages of transition back into community settings.
In order to successfully achieve both goals outlined above within their respective timelines, a comprehensive change plan must be developed which includes all members of the healthcare team as well as representatives from key areas such as finance, human resources and administration; each party should have an understanding not only how their particular role relates but also how they can contribute towards overall success.
