Urban and rural health care
The organization I selected is a rural health clinic that offers primary care services to Medicare patients. These services include preventive and routine medical care, chronic disease management, immunizations, screenings for various conditions such as diabetes or hypertension, nutrition counseling, mental health services, in-home follow up visits, and referrals to specialists when necessary.
This clinic also provides outreach programs which focus on educating the community about preventative measures for maintaining their health. For example, they may offer seminars on healthy eating habits or exercise classes geared towards senior citizens. Additionally they will provide assistance with forms such as applications for disability benefits or long-term care insurance so that the elderly population can take full advantage of available resources.
