When considering the social determinants that may be barriers to home care or case management services, it is important to take into account factors such as access to transportation, educational level, language proficiency, and income level. For example, if an individual does not have regular access to reliable transportation then they may find it difficult to attend scheduled appointments with their healthcare provider which could lead to poorer overall health outcomes. Additionally, those from more disadvantaged backgrounds who have lower education levels or limited English proficiency may not understand how to navigate the healthcare system effectively which can further complicate matters.
Furthermore, having a low income can also affect one’s ability to pay for necessary medical bills or medications which could exacerbate any existing conditions. Therefore, in order for these individuals to receive adequate home care and/or case management services it is essential that these social determinants are taken into consideration by healthcare providers in order ensure that everyone has equal access to quality care regardless of their background.
