Clinical case provided in class, develop a teaching/coaching plan
My teaching/coaching plan for this clinical case includes the following information. First, I will assess the patient’s current knowledge level by asking questions about their understanding of diabetes and medications in order to determine what areas need further clarification. Based on this assessment I will then develop educational goals that are realistic and specific such as providing weekly updates on blood sugar levels or having them attend a nutrition class at a local community center.
I will also use various strategies to deliver this information such as visual aids (i.e charts showing how different foods affect blood glucose) or role-playing different scenarios (such as discussing ways to say no when offered sugary snacks). Additionally, I will ensure that they have access to all necessary resources like support groups or online forums where they can gain additional insight from others with similar experiences. Finally, regular follow-up visits should be scheduled so that progress can be tracked over time while also addressing any questions or concerns they may have—ensuring that everyone is on the same page throughout the entire process.
