Procedures/Skills (Observed/Assisted/Performed) for Hypothetical Client:
During my clinical rotation, I had the opportunity to assist with and perform several procedures/skills on a hypothetical client.
Physical Assessment (Performed):
I performed a physical assessment on the client at the beginning of my shift. I introduced myself to the client and explained the purpose of the assessment. I checked the client’s height, weight, and vital signs, including blood pressure, heart rate, respiratory rate, and temperature. I also assessed the client’s skin color, texture, and turgor, as well as the condition of their hair, nails, and mucous membranes. Additionally, I auscultated the client’s lungs and heart, palpated their abdomen, and assessed their range of motion.
EKG (Performed):
I performed an electrocardiogram (EKG) on the client to assess their heart function. I explained the procedure to the client, positioned them on the exam table, and attached the electrodes to their chest, arms, and legs. I then started the EKG machine and recorded the client’s heart rhythm.
Vital Signs (Performed):
I performed vital signs on the client several times throughout my shift. I checked their blood pressure, heart rate, respiratory rate, and temperature. I documented the results and reported any abnormalities to the nurse in charge.
Performing these procedures/skills on my hypothetical client allowed me to gain valuable experience and improve my clinical skills. By performing a physical assessment, EKG, and vital signs, I was able to gather important information about the client’s health status and provide valuable data to the healthcare team. These procedures/skills are crucial in helping healthcare professionals to diagnose and treat illnesses and to monitor the progress of treatment interventions.
