I have selected “Inadequate Health Literacy” as the top priority problem for my community nursing diagnosis. This is due to the fact that many people in our communities lack basic health literacy skills which puts them at an increased risk of poor outcomes and can lead to higher rates of chronic disease, hospitalizations, and even death.

My program goal is to increase access to quality healthcare services by improving health literacy levels within our community. To achieve this outcome I will focus on two related objectives: 1) Increasing knowledge of medical terminology and resources; 2) Encouraging individuals to ask questions and seek out information when necessary.

To ensure that these goals are met I plan on hosting workshops and seminars with local healthcare providers, equipping public libraries with educational materials about health topics, and creating a series of online videos designed to explain common medical terms in simple language. By taking these steps we can reduce barriers associated with inadequate health literacy while empowering individuals across all socioeconomic backgrounds with the resources needed for successful self-management of their conditions.
