Based on the research, chronic pain has been found to have the greatest impact on a patient with chronic illness. Chronic pain affects a patient’s physical and emotional health, social life, and daily activities. The constant presence of pain can lead to depression, anxiety, sleep disturbances, decreased physical activity, and decreased ability to perform activities of daily living (Li, Wang, & Shi, 2019). In addition, chronic pain can also lead to social isolation, as patients may feel like they cannot participate in social activities due to their pain (Nikolajsen, Jensen, & Schrøder, 2017).
When comparing the impact of chronic pain on a twelve-year-old versus a seventy-five-year-old, there are some differences. For a twelve-year-old, chronic pain may impact their ability to participate in physical activities, such as sports or playing with friends. It may also affect their school attendance and academic performance. On the other hand, for a seventy-five-year-old, chronic pain may affect their ability to perform activities of daily living, such as bathing, dressing, or cooking. It may also lead to social isolation, as they may be unable to participate in activities with friends or family.
The chronically ill patient’s illness trajectory influences the plan of care by providing information on the expected progression of the disease and the patient’s needs at different stages of the disease. For example, if a patient’s illness trajectory indicates that they will have frequent exacerbations, the plan of care should include strategies for managing these exacerbations, such as having medications readily available or having a plan in place for when to seek medical attention. In addition, the patient’s illness trajectory can also help identify potential complications of the disease, which can be addressed through preventative measures.
The Healthy People 2030 goals focus on improving the health of the nation by addressing specific health issues, such as mental health, substance abuse, and chronic diseases. As a nurse, incorporating these goals into my day-to-day practice can involve educating patients on healthy behaviors and lifestyle changes that can help prevent chronic diseases, such as healthy eating habits, regular physical activity, and smoking cessation. In addition, I can also promote mental health by identifying patients who may be at risk for mental health disorders and providing resources for counseling or support groups. By addressing these issues in my nursing practice, I can contribute to improving the overall health of the nation.
