To develop a comprehensive care plan, it is important to identify the health risks faced by the aggregate. Based on the risk assessment, two priority-nursing diagnoses can be proposed. For example, if the aggregate is at risk for diabetes and hypertension, the priority nursing diagnoses could be:
- Risk for unstable blood glucose levels related to poor dietary habits and sedentary lifestyle.
- Risk for ineffective management of blood pressure related to poor medication adherence and lack of knowledge on lifestyle modifications.
To address these nursing diagnoses, strategies can be developed. For instance, for the first diagnosis, strategies could include providing education on healthy eating habits and increasing physical activity levels. For the second diagnosis, strategies could involve providing medication education and counseling on lifestyle modifications such as salt reduction, increased physical activity, and weight loss.
To support these strategies, at least two journal articles can be used. For example, an article titled “Effectiveness of Lifestyle Modifications in the Prevention and Management of Hypertension” by Brown et al. (2019) could be used to support the second nursing diagnosis. Another article titled “Dietary Habits and Risk of Type 2 Diabetes Mellitus in a Rural Community in Southern Thailand” by Wangtongkum et al. (2017) could be used to support the first nursing diagnosis.
In addition to the nursing diagnoses and strategies, a disaster management plan can also be developed. The plan could include a list of disasters that might affect the aggregate, strategies for handling at least two disasters from the list, and recommendations for a disaster supplies kit. For instance, if the aggregate is located in an area prone to hurricanes, the disaster management plan could include strategies such as evacuation plans, establishing a communication plan, and having a well-stocked emergency supplies kit.
Finally, it is important to cite all sources using APA format on a separate references page. The title and reference pages should not be included in the total page count of the paper.
