As nurses, we have a crucial role in ensuring that we provide quality patient care and support the improvement of the health care system. To do this, we must keep ourselves updated on the latest developments in our field, particularly in health care reform.
One of the significant concepts that we need to be familiar with is the continuity or continuum of care. This refers to the coordinated and seamless delivery of health care services across different settings, including hospitals, clinics, and communities. It means that healthcare providers, including nurses, need to work together to ensure that patients receive the appropriate care and support throughout their health care journey. Continuity of care has been linked to improved patient outcomes and reduced costs of health care.
Another concept that we should be aware of is the accountable care organization (ACO). ACO is a group of healthcare providers, including hospitals, physicians, and nurses, who work together to coordinate and improve patient care while lowering health care costs. ACOs aim to promote the delivery of high-quality, cost-effective healthcare through coordinated care, patient-centeredness, and quality measurement.
Medical homes are another concept that we need to know. Medical homes are primary care practices that provide comprehensive and patient-centered care. Medical homes focus on building strong relationships between patients and their healthcare providers, including nurses. The goal is to provide personalized care that is accessible, coordinated, and effective.
Lastly, nurse-managed health clinics are healthcare facilities run and managed by nurses. These clinics offer primary care services to underserved communities, particularly those who have limited access to healthcare. Nurse-managed health clinics aim to improve health outcomes, reduce health disparities, and lower healthcare costs.
