The nurse was responsible for coordinating the care that was provided. The nurse’s role in this process included assessing the patient’s needs, developing an individualized plan of care based on these needs, communicating with other members of the health care team to ensure all required services were being addressed, and monitoring ongoing progress towards desired outcomes. The nurse also had to be cognizant of any changes or updates that occurred during treatment as well as any potential safety concerns that may arise so they could take necessary action when needed. To ensure proper coordination of care, nurses must have a thorough understanding of the patient’s medical history, current condition, and their specific goals in order to develop an effective plan of care.
