For example, when assessing health issues related to diabetes, nurses would first be tasked with collecting data from sources such as medical records or interviews with patients. This information would then be used to diagnose the problem by identifying any risk factors or contributing behaviors associated with the condition. After diagnosis is complete, nurses can then begin planning for appropriate interventions based on research literature and best practices for diabetes management. Finally, nurses will implement their plan of care and evaluate its effectiveness by monitoring patient progress against predetermined goals thus allowing for necessary adjustments in order to ensure optimal outcomes.
