Subjective data: The patient reports having difficulty breathing for the past few days as well as an increase in wheezing over the last week. She also mentions feeling more fatigued than usual and has noticed that her lips are often dry.
Objective data: Vital signs reveal tachycardia (heart rate of 120 bpm) and increased respiratory rate (25 breaths/min). Physical exam shows clear lungs with decreased air entry on the right side. Blood sugar is elevated at 140 mg/dL.
Summary of positive findings in history: The patient’s medical history suggests that she has been managing her chronic illnesses relatively well over time as she has been able to keep her blood pressure under control and her blood sugar levels within normal limits despite having diabetes. Additionally, there have not been any major exacerbations related to her asthma prior to this episode which could be helpful in determining what might be causing her current symptoms.
