During the physical examination, I found her blood pressure to be elevated at 150/90 mmHg and her BMI was 35 kg/m2 indicating obesity. Her heart rate was regular without any murmurs or extra sounds heard during auscultation . Her respiratory rate was 12 breaths per minute and lungs were clear bilaterally with good air entry . Additionally , abdominal exam revealed normal bowel sounds present along with soft rounded abdomen without any palpable masses
Based on my findings , I formulated diagnosis of Hypertension (HTN) & Overweight (OW). For HTN, I prescribed lifestyle modifications such as increasing activity level , reducing salt intake & monitoring blood pressure daily while OW required dietary interventions like reducing caloric consumption by eating smaller portions over more frequent meals while avoiding highly processed foods whenever possible .
In conclusion , this health assessment check off project enabled us gain better understanding about how particular condition affects individual patient thus allowing us devise necessary treatment plans according their unique needs thereby improving chances success when it comes managing diseases such HTN or OW going forward.
