The next step would be to conduct interviews with the staff involved in the situation as well as any relevant patients who were present. During these interviews, I would ask for examples of how the problem was handled before and after the incident occurred. By getting feedback from those directly involved, I can compare stories and get better insight into whether this was an isolated issue caused by one nurse’s actions or if it was part of a larger systems problem that needs to be addressed company-wide.
With every aspect considered, I can determine whether this issue came from lack of training or oversight on behalf of our organization, inadequate staffing resources, unclear protocols causing confusion among our staff, etc., which signals a need for changes within our system itself; or if it resulted from individual negligence on behalf of one nurse’s tasks or responsibilities required of them by their job description and expected level of performance. With this information gathered together and analyzed properly we can then make decisions about how best to proceed forward either at an organizational level or with regards to disciplining an individual nurse appropriately according to policy regulations set forth by our institution as necessary.
