Week 5 community practice experience journal reflection
1. Develop a comprehensive plan for working with clients to address their health and social service needs.
2. Attend relevant trainings on case management skills and knowledge related to the client’s individual case management circumstances.
3. Monitor progress of the client’s treatment, recovery, and rehabilitation goals through regular contact with medical professionals, support services providers, and other stakeholders in the client’s life.
4. Identify resources within the community that may be beneficial to clients in meeting their health or social service needs or achieving their treatment goals.
5. Develop relationships with professional networks and agencies that can provide access to additional supports needed by clients during their recovery process.
6. Facilitate successful connections between clients and appropriate external resources or services when necessary or requested by clients or authorized agents of theirs such as family members or legal representatives.
7. Prepare accurate documentation of all activities related to a client’s care including assessments, interventions, referrals, resources provided, outcomes obtained, etc., which will be used in compiling reports for funders regarding program compliance standards and effectiveness measurements
