1. Physical Assessment: This would include assessing the patient’s vital signs, body systems, and any physical limitations or impairments that may be impacting their ability to care for themselves.
2. Psychosocial Assessment: Evaluating the patient’s mental health status and social support system, in order to identify any potential psychological issues that could impact their overall quality of life.
3. Cognitive Assessment: Assessing the patient’s cognitive abilities such as memory, concentration and problem-solving skills in order to determine their level of functioning and ability to make decisions about their care.
4. Self-Care Needs Assessment: Identifying the patient’s individual self-care needs such as diet, exercise regimen, medication requirements and activities of daily living (ADLs), in order to develop an appropriate plan of care for them.
