These include the use of standardized best practices for patient care, ongoing education for staff on how to identify and address potential hazards, implementation of an incident reporting system that allows employees to confidentially report near-miss events, 24/7 risk management teams which are responsible for investigating any reported incidents and taking corrective action, mandatory safety checks prior to performing procedures or administering medications, frequent quality assurance reviews by outside experts or organizations, increased collaboration between medical personnel in different departments to ensure consistency in patient care processes. Additionally, health care providers are now being held accountable with financial penalties if they are found responsible for any never events.
