The LPN and RN should collaborate to develop the nursing plan of care by assessing the patient’s individual needs, evaluating past interventions, and developing potential solutions. The LPN can provide essential information related to a patient’s medical history and health condition while the RN is responsible for creating a comprehensive care plan that meets the patient’s needs. Effective collaboration between an LPN and RN will help ensure that all areas are considered when designing a plan to reach each goal. This includes choosing appropriate medications, coordinating education sessions with other healthcare professionals (e.g., dietitian), involving family or other support systems, devising strategies for managing side effects or relapse, obtaining necessary resources from outside agencies, scheduling follow-up visits, monitoring progress through regular assessments, providing emotional support/counseling as needed and referring patients to other specialists or services when necessary.
