The actions of people and organizational leaders directly and indirectly involved with the tragedy can be highly unethical and even criminal. Some examples include: executives falsely claiming safety measures were in place when they were not, failure to provide professional training or adequate supervision for staff members, ignoring complaints from employees about unsafe conditions, lack of oversight over a contractor responsible for constructing the building, withholding information from investigators or victims’ families, refusal to accept responsibility for their errors which led to an avoidable tragedy. The ethical issues these individuals faced include gross negligence, breach of fiduciary duty, wrongful death/injury liability, fraud/misconduct in office, usurpation of public resources for private gain.
