Patient SMART Goal: By the end of 3 months I will have achieved a 10% reduction in my blood sugar levels.
Evidence-Based Plan of Care:
1. Follow a balanced diet consisting of nutrient-dense foods such as fruits and vegetables, lean protein sources, whole grains, and healthy fats.
2. Exercise regularly for at least 30 minutes per day 5 days per week by engaging in activities such as walking or cycling.
3. Monitor fasting blood sugar levels weekly to monitor progress towards goal and adjust nutrition and exercise plan if necessary
4. Take medication prescribed by physician as directed
5. Create an action plan with healthcare provider to address any important life changes that may interfere with achieving goals (e.g., quitting smoking, reducing stress)
6. Attend follow up appointments with healthcare team to evaluate progress
