For the evaluation plan, a data collection process and analysis of the efficiency, efficacy and effectiveness of fall prevention strategies as an intervention in hospitals will be used. The evaluation criteria will include pre-post self-report surveys from patients and family members on falls acquired before and after implementation.
Quality improvement charting for monthly reports to measure safety outcomes associated with fall incidents will also be implemented. Observations by staff members during rounding sessions regarding any changes in behavior post implementation along with focus group interviews to gain insight into perceptions of staff related to their views on implementing the change proposal are also part of the evaluation plan. Additionally, data tracking forms for capturing patient information about compliance with protocol steps such as consulting a physician or nurse if feeling unsteady while showering or ambulating outside his/her room is monitored.
By implementing this comprehensive evaluation process which tracks multiple factors related to each objective it would help detect any discrepancies between expectations/goals set out when formulating the proposed change that was put forward at outset in order determine degree success achieved thereby providing necessary information gauge progress made towards achieving overall end goal preventing falls hospital settings across board regionally differently viewed according specific needs populations served accordingly.
