Dashboard analysis and nursing plan
When analyzing the Sample Dashboard from this week’s resources, I have chosen falls prevention as one area that needs improvement for my nursing plan. According to the sample dashboard data provided by NDNQI, our hospital’s actual rate fell above or below benchmark only 54% for post-fall incidents observed between January 2018 through December 2018 (Dashboard Sample 2020). In other words, our actual rate was 6 or more points higher than expected which indicates good opportunities for improvement. As per evidence based practice literature from sources such as The Joint Commission (2020), there are several strategies that can be implemented by healthcare providers such as staff education/training programs regarding fall assessment risks and protocols; increased supervision of high-risk patients; improved environmental safety; using assistive devices if needed when mobilizing patients etcetera.
Therefore my suggested nursing plan includes five best practices principles with potential solutions related specifically towards reducing incidence rates of falls amongst hospitalized patients:
Firstly daily risk assessments must conducted upon admission – with follow up intervals appropriate nature patient – order evaluate promptly any changing conditions causing increases likelihood past lessons learned highlighted certain levels thorough reports maintained examine detail yearly basis point view making informed decisions prescribed treatments medications depending reactions alone taken into account formulation universal policy ensure enacted effect combatting raised alarms assumptions drawn carefully accordance findings discussed earlier times happened reason linked firmly undeniable proof done fact discussions chaired relevant departments situation type added detail avoidance mechanisms place modifiable constantly monitored setting wise atmosphere pervades corporation afterwards every part operations smoothly running wheel appears never stop happen due diligence .
