This requires engaging with team members on all levels—including nurses, patient care assistants, social workers, unit leaders, etc.—to brainstorm ideas that can reduce avoidable ED visits without sacrificing quality of care. By encouraging ownership over the problem solving process and gathering suggestions from frontline staff who are most likely experience with patients’ day-to-day needs at first hand – new interventions & protocols can be developed & designed specifically for nursing home setting targeting reduction within short term [2]. The leader must also ensure open channels of communication between different departments so any proposed changes are communicated clearly before implementation [3][4] .
If done successfully ,this could help break down silos existing across entire system while building proactive action plans focusing both proactively intervening early when needed while staying away already congested emergency rooms unless absolute necessity required[5][6]
