The model of nursing care observed in the practice setting was a team-based model. This model focused on collaboration between nurses, physicians, and other members of the healthcare team to provide optimal patient care. The team-based approach emphasizes communication and shared decision making in order to provide high quality, safe, and effective care for patients. Members of the healthcare team worked together to review each patient’s case at daily rounds and develop an individualized plan of care tailored to their needs. During my observation I saw nurse practitioners and physicians working collaboratively with one another as well as nurses taking initiative by providing education about treatments or medications before discharge or follow up visits. Nurses were also involved in performing assessments when needed such as reviewing lab results or observing changes in patient condition over time. There was a clear focus on understanding the patient’s individual needs which allowed for more comprehensive treatment plans that incorporated elements from multiple disciplines within the medical field including pharmacology, social work, dietician services etc.. In addition there seemed to be seamless integration between various levels of nursing staff such as LPNs/LVNs float RNs etc., allowing for better utilization resources while delivering quality care without sacrificing efficiency due limited productivity among certain positions if they were divided into separate roles instead . All in all this approach allows for better coordination between all parties involved resulting greater continuity across shifts which is beneficial not only from a clinical standpoint but it can improve morale among staff members since everyone feels like they are part of something bigger than themselves rather than just feeling isolated or disconnected from their peers at work .
