Introduction to the Project
This Nursing Plan project uses the Nurse-Sensitive Q Indicator Dashboard SCORE performance results. It aims to assess the data in the dashboard for the Adam 5 inpatient rehabilitation facility to determine the areas of high performance and the ones that need improvement. This project proposes various methods of improving the performance in an opportunity area using a select indicator of one area. It is based upon at least three nursing best practices that are evidence-based and correlate with the health care performance improvement model.
Part II: Potential and strengths in performance
Every quarter, every Nursing Care section in an institution is required to meet predetermined SCORE performance objectives. SCORE, an assessment tool used to track the performance in a specific facility area, is Brown et al. (2008). These outcomes can be used as indicators to determine the strengths and weaknesses of an organization’s health care delivery system. Based on the results of the analysis, the facilities create a nursing program as an important strategic development in order to improve its strengths and address their weaknesses. It is vital for continuous improvement of the patient’s services.
NDNQI data for Rehabilitation, inpatient care unit indicated that several areas achieved high SCOREs that were higher than the target margins. R.N. was one of the good performance categories. The care hours of patients with a higher SCORE variation (Q2 to Q4) increased from +3.15 percent in Q1 to 7.79 percent during the fourth quarter. It is clear that staff management strategies work. Functional Independence of the Bowel margins of the patient indicated that the patients were able to process more food through their digestive tracts. R.N. was another area that showed exceptional results. Courtesy, LOS (Length Of Stay) were two other areas that showed exceptional performance.
However, they have the potential to be successful if they are unable to meet performance SCORE goals. This included the R.N. These included the R.N. Patients with Ulcers required immediate care.
