My practical experience revealed that there was a lack of communication between healthcare workers caring for patients. Sometimes, the complications that could arise from inconsistencies when turning the procedure over can also affect the health of seriously ill patients. Misunderstandings between medical personnel sometimes caused delay in investigations and consultations with specialists. Miscommunications or misunderstandings can have negative effects on the patient’s outcome and increase practice risk.
Patients safety supporters agree that good communication is essential to ensure effective healthcare. Interprofessional teamwork is essential for providing safe, patient-centered treatment in the healthcare setting (Lee & Doran, 2017). Effective communication between multidisciplinary teams is crucial for teamwork and coordination. There is substantial evidence to support the idea that communication can play an important role in creating a positive culture. This in turn will benefit the patient. This knowledge increases the effectiveness and safety of care. Emancipation gives nurses the ability to promote patient safety. Team initiatives promote and sustain a culture of safety. This in turn contributes to the improvement of quality care.
Families and patients need to be involved in order for adverse outcomes to be reduced, treatment can be safe, and safety culture fostered. Understanding the needs of patients and families and incorporating them into the creation of a care plan will improve their knowledge of diagnosis, treatment, procedures and testing. Interprofessional communication is crucial for patient safety and improved clinical performance. An RN’s primary purpose would be to ensure that patients and providers are in constant communication regarding the treatment plan. It will be a significant improvement in the client’s health if panels are constructed and surrounding areas encourage regular communication between the patient care team members.

The imaging results for Bronchiolitis (and pneumonia) are very similar. A CT scan may show Bronchitis patients who have centrilobular nodules and air trapping. Apart from the centrilobular microdules, dilatation of the bronchi and air trapping, there are other non-specific observations such as micronodules in the centrilobular, dilatation of bronchi and air trapping.
COPD chest imaging shows increased radiolucency in the lung fields. It also shows an expanded retrosternal area, diaphragm sagging, wide intercostal gaps and constricted cardiac space.
